Player Satisfaction Surveys will be distributed the weekend of Feb 4th 2011
The “Players Satisfaction Surveys” will be distributed to division Convenors by Feb. 4/2011 and in turn distributed to your respective teams. They will have the Division, Team Name and Coaches name already printed on the forms this season. Please speak to your parent rep or head coach to receive your form. If you do not receive a copy then please print one out from the following link:

Your feedback is very important to the function of the CMHA and we appreciate your efforts in filling out the forms. If you do not feel comfortable submitting your survey to your coach please give the form directly to Jerrold Labrecque at the weekend games.
